Aboriginal and Torres Strait Islander health
Integrated Team Care is provided by teams of Indigenous Health Project Officers, Aboriginal and Torres Strait Islander Outreach Workers and Care Coordinators. The teams work in the Brisbane South PHN region, across both mainstream and Aboriginal and Torres Strait Islander primary healthcare sectors, to assist Aboriginal and Torres Strait Islander peoples to:
- obtain primary health care as required
- provide care coordination services to eligible Aboriginal and Torres Strait Islander peoples with chronic condition/s who require coordinated, multidisciplinary care
- improve access for Aboriginal and Torres Strait Islander peoples to culturally appropriate mainstream primary healthcare.

Important update regarding the Closing the Gap program
From January 2017 the Institute of Urban Indigenous Health (IUIH) will commence delivery of the Brisbane South PHN region’s Closing the Gap program.
Services including client support, general practice education and information on cultural awareness, MBS item numbers and PIP eligibility will continue as normal.
